After more than 30 years of bringing people together to improve community health in the Pikes Peak region, Community Health Partnership (CHP) has adopted a new name: ChangeLine.

The rebrand marks the next chapter for the nonprofit, which was founded in 1992 to improve coordination among local healthcare providers. Over the years, in response to community needs, its mission expanded to the root causes of complex challenges like suicide prevent ion, homelessness, behavioral health, and LGBTQ+ health equity.

“We’ve learned that solving these challenges requires people to work together in a different way,” said Amber Ptak, Chief Executive Officer of ChangeLine. “That realization has shaped how we show up – and ‘ChangeLine’ better reflects who we are today: a civic hub that helps communities lead change together.”

As a civic hub, ChangeLine is a conduit for community-powered solutions, helping create the conditions needed to drive meaningful, lasting change. It’s not just about bringing people together – it’s about building the trust, structure and shared strategy that turn collective effort into long-term impact.

“Transformative change doesn’t happen by accident – it takes intention and investment,” said Ptak. “Our role is to help communities build the civic infrastructure – the relationships, processes, and shared purpose – that drives real progress.”

As part of this transition, new websites were created for both ChangeLine and the Pikes Peak Continuum of Care (PPCoC), a program of ChangeLine. Previously housed on the CHP website, the new standalone PPCoC website brings it into alignment with the state’s three other CoCs. It also helps position the PPCoC more visibly as a driving force for strong, human-centered solutions to homelessness in the Pikes Peak Region.
